The Conference Organizers recognize the critical importance of facilitating the attendance of
persons living with HIV/AIDS, representatives of nongovernmental organizations, community-based
organizations, health departments, AIDS service organizations, and academics, including college/university
students who would not otherwise be able to attend this conference. In an effort to increase
attendance from these important groups, the Conference Organizers are committed to soliciting
government organizations, associations, foundations, and corporations to support the Scholarship
Program. Priority will be given to individuals whose participation at this conference will
clearly enhance their prevention efforts, and to persons who will have the opportunity to
share the skills and knowledge they acquire at the conference. Because this conference is
science-based, scholarship awardees will have the unique opportunity to learn about scientifically
proven HIV prevention strategies from nationally recognized experts. The number of scholarships
will depend on available funding. No more than two applications per agency will be accepted. International
applications will be considered on a case-by-case basis; however, priority will be given to domestic
applicants. All applicants must be at least 18 years of age.
The deadline for submitting scholarship applications is Friday, April 8, 2005. Individuals wishing
to apply for a scholarship must complete the Scholarship Application Form and return it with all
required attachments by postal service (postmarked no later than April 6, 2005) or by courier service
(e.g. Federal Express, etc.) no later than April 8, 2005. You will receive a confirmatory e-mail
or phone call indicating that your application was received by the Scholarship Coordinator within
2 weeks of submitting the application. You may want to call to confirm that your application has
arrived if you do not receive this confirmation within 2 weeks.
The information that you provide on the Applicant Profile and Applicant Information forms is considered confidential and will be shared only with the staff and organizers of the 2005 National HIV Prevention Conference for the purpose of reviewing and selecting scholarship recipients. All essays will be reviewed by two different reviewers. Scholarship applicants will be notified by May 2, 2005, of the results of their scholarship application submissions. The total number of scholarships available has not yet been determined, but will be limited. Therefore, all applicants are strongly encouraged to seek funding from other sources in addition to applying for a scholarship.
1. Complete and submit the Application Form.
2. For individuals who work in the HIV/AIDS field (paid or volunteer), a letter of support from your organization (with a contact person and telephone number) must be attached to your application.
3. Attach a copy of your abstract, if you have submitted one to the conference.
Please submit one application form per individual per organization, agency or group (a maximum of two applications per group). The essay portion of the application must be complete in order to be considered for a scholarship award. Faxed or e-mail application submissions will not be accepted. Applications that are incomplete or are postmarked after April 6, 2005 will not be accepted. This application may be photocopied.
