Registration received before March 30, 2005$300.00
Registration received before April 27, 2005$350.00
After April 27, 2005, registration on-site only$400.00

Payment

Fees should be paid in US dollars by check, money order, or credit card (MasterCard, VISA, American Express). Checks should be made out to CISTEMS. Registration fees must accompany the conference registration form.

Cancellation / Refund

Written cancellations that are postmarked by May 4, 2005, will be honored. Refunds less a $50 administrative fee will be returned 6-8 weeks following the conference. Cancellations received after May 4, 2005, will forfeit 100% of the registration fee.

Registration should be submitted ON-LINE on the conference web site or by mail on the official registration form and can be downloaded HERE (photocopies are acceptable). No faxed registrations will be accepted! Mail registration forms with payment to:

Abstracts may be submitted without a registration form and payment. Authors whose submissions have been accepted will be expected to register and pay the conference registration fees.


You will receive confirmation of registration by mail or e-mail. Registration is limited by the size of the conference venue, therefore on-site registration cannot be guaranteed. Please check the web site at after April 27, 2005, to confirm whether on-site registration will be available.
