Payment

Fees should be paid in US dollars by check, money order, or credit card (MasterCard, VISA, American Express). Checks should be made out to CISTEMS. Registration fees must accompany the conference registration form.

Cancellation / Refund

Written cancellations that are postmarked by May 4, 2005, will be honored. Refunds less a $50 administrative fee will be returned 6-8 weeks following the conference. Cancellations received after May 4, 2005, will forfeit 100% of the registration fee.

Registration should be submitted ON-LINE on the conference web site or by mail on the official registration form and can be downloaded HERE (photocopies are acceptable). No faxed registrations will be accepted! Mail registration forms with payment to:

Abstracts may be submitted without a registration form and payment. Authors whose submissions have been accepted will be expected to register and pay the conference registration fees.


You will receive confirmation of registration by mail or e-mail. Registration is limited by the size of the conference venue, therefore on-site registration cannot be guaranteed. Please check the web site atafter April 27, 2005, to confirm whether on-site registration will be available. Continuing Education Credits

The conference organizers will be applying for continuing education credits for interested participants through the CDC's authority to grant credits. If the conference is approved, the following credits will be offered; Continuing Medical Education (CME) credits, Continuing Nursing Education (CNE) credits, Continuing Education Units (CEUs), and Certified Health Education Specialist (CHES) contact hours. These credits will be awarded free of charge.

Scholarships

The conference organizers are working to establish a scholarship process that supports individuals requiring additional assistance. Please check the web site at after January 10, 2005, for further information.

Conference Material

Upon presentation of your receipt of registration at the conference check-in area, you will receive the conference program book, an official conference badge, and other materials.

Lodging

Conference participants will need to make their own lodging arrangements. The main conference hotel (Hyatt Regency Atlanta) and other nearby hotels will maintain a block of rooms for conference participants until May 11, 2005. Conference registrants should call before this date, identify themselves as participants at the 2005 National HIV Prevention Conference, and reserve a room with a credit card. Telephone numbers are listed below; hotels charge the prevailing per diem rate plus tax.

Badges

Official conference badges must be worn at all times to gain and maintain access to conference venues. Proof of registration and identity will be required to receive your conference badge. A charge will be incurred for replacement badges.

Special Interest Meetings

A limited number of meeting rooms will be made available to conference participants for scheduling meetings of special interest groups. Meeting space is available before or after scheduled conference meetings. Rooms will be assigned on a first-come, first-served basis, beginning in May, 2005. A meeting schedule book will be kept at the registration desk. Please check the web site for updated information.

Exhibition Space

During the conference, exhibit space will be available for lease by commercial and nonprofit organizations. Space will be available for a fee on a first-come, first-served basis. Please check the web site at for further information. Prior to the conference, updated information about exhibit space will be posted on the conference web site.

NGO Village

On Monday, June 13, 2005 from 6:00 p.m. through 8:00 p.m., non-profit non-governmental organizations can showcase their HIV prevention efforts free of charge. Please visit the web site, www.2005HIVPrevConf.org, for additional information. NGO table reservation forms are due in May, 2005 along with proof of nonprofit status; space is limited and will be assigned on a first-come, first-served basis.
